Our AOE/COE program has been developed from years of experience.  AOE/COE investigation requests are immediately assigned to an associate investigator to review and acknowledge receipt of the assignment to the referring agency.  Before meeting with the employee, employer or witness, a detailed statement and/or interview outline addressing the type of injury alleged [e.g., specific injury, cumulative trauma injury, psychiatric injury, occupational disease or illness], is prepared.   The format is designed to elicit pertinent information including, but not limited to, personal history, facts and circumstances relating to the claimed injury, causation, medical treatment [past, present and future], first notice of the injury, contributing factors, lost earnings, anticipated future lost earnings, medical history, employment history, legal consultations, prior accidents, pre-existing conditions, and prior claims.

Issues relevant to affirmative defenses and judicial doctrines are addressed to assist the claims administrator and their medical/legal personnel in determining the compensability of the alleged industrial injury pursuant to the requirements set forth in the Labor Code.

At the time of taking the applicant's statement, the applicant is read and asked to acknowledge his or her understanding of a felony-fraud advisement.  The applicant's identification photograph is taken for future reference and written authorizations for release of pertinent records are obtained.  Depending upon the claim, the authorizations may include medical releases, psychiatric releases, personnel records release, alcohol/drug/HIV releases, and/or insurance company record release.
